Notarization in a Medical Facility
If a notarization is needed for someone in a Hospital, Nursing Home, or Long Term Care Facility I am aware that the situation may already be difficult or stressful. The goal is to make the signing experience as stress-free and as easy as possible. To accomplish this, there are some things you need to know to ensure a smooth and easy signing.
- The signer must be alert and aware
- All medications that would inhibit cognitive awareness must be with held 1 hour before arrival
- If any version of pressure, coercion is suspected, the room must be cleared of all non-medical personell
Signature By Mark
If the signer is unable to sign their name, the mark serves as the signature.
For each signature by mark to be notarized, there must be 2 witnesses to the making of the mark, in addition to the Notary. Both witnesses must sign the document. The Notary must write out the marker’s name beside the mark. A mark must also be made in the Notary Journal, and the witnesses must sign as well.
